In high relief, bareheaded bust of Verwoerd in jacket and tie, front. Legend on a raised band above: “DIE EERSTE MINISTER” and below between ornate stops: “DR. H.F. VERWOERD”.

Large numeral: “5”. Above: “DIE REPUBLIEK (in an arc) / VAN / SUID AFRIKA” and lower down round the rim on each side: “1961 (left) 1966 (right)”. Silver medal stamped: “SILVER” at the foot.

Hendrik Frensch Verwoerd (1901 - 1966) was a member of the National Party of South Africa and Prime Minister from 1958 until his assassination in 1966. Under his premiership South Africa broke away from Great Britain and became a republic in 1961. He is remembered as the man behind the conception and implementation of apartheid.
It is reported (AM2) that a total of 1050 medals in gold, silver and bronze were made, although not how many of each metal.
