Edge: Plain. Stamped at 6 o'clock with the maker's mark: "ET+" and "18CT (springbok head) S (for 1966)" and at 12 o'clock with the issue number. A seen medal pair: "1952". A seen singe medal: "0347"

Obverse: A stylised design of two children (5-year olds) playing with a spilling cornucopia and behind a leaping figure, left, right arm extend, holding baton pointing upwards. Signed: "CLS" below, left.

Reverse: Within a stylised laurel wreath, a large figure: "V". Across on left: "1961" and on right: "1966". Legend above: "REPUBLIEK VAN SUID AFRIKA / HERDENKING" and below: "ANNIVERSARY / REPUBLIC OF SOUTH AFRICA".

Notes: Issued in three different sets:
(1) As a set of three medals (43 mm, 32 mm and 21 mm) inside an oblong black fitted case lined on the inside with white silk and red velvet. On the outside of the lid in gilt, left side: "THE REPUBLIC / OF / SOUTH AFRICA / FIFTH ANNIVERSARY MEDALLIONS" and on the right side, separated by a line: "DIE REPUBLIEK / VAN / SUID-AFRIKA / VYFDE / HERDENKING- / MEDALJES".
(2) As a set of two medals (32 mm and 21 mm) in a shorter oblong case but otherwise identical.
(3) The single 21 mm medal in a square black fitted case lined on the inside with white silk and blue velvet. On the outside of the lid in gilt, a reproduction of the medal's reverse.

A numbered (same as the medals) certificate from the South African Bureau of Standards and the manufacturer, E. Thiessen, is included giving the mintages.
