Obverse: A photographic image, held in the central medallion, of four British Generals, each named: Roberts (top), Buller (right), Kitchener (bottom) and White (left). Impressed on the arms of the cross, shaped like clover leaves, rose (top), shamrocks (right), Irish harp (bottom) and thistle (right). On the suspension bar: "CANNOCK & HEDNESFORD / DISTRICT COMMITTEE".

Reverse: Across: "IN COMMEMORATION (in an arc) / OF THE / RELIEF OF / LADYSMITH.. FEB. 28. / SURRENDER OF / CRONJE / 27 FEB. 1900 (in an arc)". Named: "FENWICK (left) B'HAM (right)".

Notes: Hednesford and Cannock are adjoining towns in Staffordshire, England.

The dies for the cross and the suspension bar were also used to make a cast silver medal. The pictorial image is not present and the central medallion has been reversed and silver plated. This appears to have been presented as a tribute medal from the Hednesford and Cannock District Committee.
