On a concave pebbled background, Springbok leaping to the left with two crossed protea flowers below. On a raised band above: “REPUBLIC OF SOUTH AFRICA” and below between stops: “1961 – 1966”.

A large Roman numeral "V" superimposed on a map of South Africa. On a raised band above: “REPUBLIEK VAN SUID-AFRICA” and below between stops, two crossed sprigs of laurel.

Issued individually in a blue case lined with white silk and blue velvet. Some of the cases are inscribed on the inside of the lid: "Medal Art (in quotes) / (Pty.), Ltd. / (Edms.) Bpk. / Box/Bus 1483 PRETORIA".
It is reported in AM2 that the production of these medals was stopped because sufficient numbers would not have been ready for the May celebrations. Only about two hundred sets were produced before the stoppage.
