Obverse: Coat of arms of Cradock, ostrich feather on each side, cornucopia crest, with motto on a ribbon below: "PERSEVERANTIA VINCIT". Legend on a raised roughened band: "MUNISIPALITEIT.CRADOCK.MUNICIPALITY."

Reverse: Within a laurel wreath: "CRADOCK (in an arc) / (space) / 1814-1964 (in an arc). Silver medal stamped with SA Mint hallmarks for sterling silver: "(lion head) STG Q (for 1964) S.A.M".

Notes: Cradock is a town in the Eastern Cape Province, founded in 1818. It was named after Sir John Cradock, governor of the Cape from 1811 to 1813.
